Aspiration Pneumonitis by regurgitation during Laryngoscopy in the Patient with mechanical Ileus - a case report-



Abstract
Aspiration pneumonitis is one of the most serious complications of general anesthesia. Intestinal obstruction, anatomicla changes of airway, extremes of age, pregnancy, and emergency operation are predisposing conditions of aspiration
pneumonitis.
Forty-six year-old male patient underwent band lysis under the diagnosis of mechanical ileus. During laryngoscopy for endotracheal intubation, gastric contents in the oropharynx and larynx were observed. Arterial oxygen saturation detected by
pulse
was
maintained 90 to 95% during general anesthesia. After the operation, patients showed tachypneic(30~40rates/min), tachycardiac(120~125 beats/min) and febrile(38.3¡ÆC) condition. We made a diagnosis of pulmonary aspiration and secondary bacterial
pneumonitis, and successfully treated him for aspiration pneumonitis with mechanical ventilator with PEEP(8!12 mmHg), antibiotics and other conservative treatments.
